INDICATION

Zyto 250mg Tab is a macrolide antibiotic indicated for mild to moderate bacterial infections including community-acquired pneumonia, pharyngitis and tonsillitis caused by Streptococcus pyogenes, acute exacerbations of chronic bronchitis, sinusitis, skin and soft tissue infections, and urogenital infections caused by Chlamydia trachomatis.

MECHANISM OF ACTION

Azithromycin binds reversibly to the 50S ribosomal subunit of susceptible bacteria, blocking translocation and inhibiting RNA-dependent protein synthesis. It has a unique pharmacokinetic profile with a very long tissue half-life of 68 hours and extensive tissue distribution, producing high and sustained drug concentrations at infection sites after short treatment courses. The 6-tablet pack provides the standard 5-day z-pack regimen (500mg on day 1, 250mg on days 2 to 5) in a convenient quantity.

DOSAGE AND ADMINISTRATION

•      Standard 5-day z-pack: 500mg (two 250mg tablets) on day 1, then 250mg (one tablet) once daily on days 2 to 5

•      Chlamydia urogenital infection: 1g (four 250mg tablets) as a single dose

•      Take with or without food

•      Always complete the full prescribed course even if symptoms improve

SIDE EFFECTS

Common:

•      Nausea, vomiting, diarrhoea

•      Abdominal pain

•      Headache

•      Dizziness

Less common:

•      QT prolongation and cardiac arrhythmias (risk in at-risk patients)

•      Clostridium difficile-associated diarrhoea

•      Abnormal liver function tests or cholestatic jaundice

•      Hypersensitivity reactions including rash and angioedema

NOT RECOMMENDED IN

•      Known hypersensitivity to azithromycin, erythromycin, or macrolide antibiotics

•      History of cholestatic jaundice with prior azithromycin

•      Patients at risk of QT prolongation or on QT-prolonging drugs

•      Severe hepatic impairment

DRUG INTERACTIONS

QT-prolonging drugs including antiarrhythmics, fluoroquinolones, and antipsychotics increase arrhythmia risk. Antacids reduce azithromycin absorption rate; take 1 hour before or 2 hours after antacids. Warfarin anticoagulant effect may be enhanced; monitor INR. Cyclosporine levels may increase; monitor closely.
