INDICATION

Zilero 2.5mg Tab is a direct oral anticoagulant (DOAC) and factor Xa inhibitor indicated for the reduction of stroke and systemic embolism risk in patients with non-valvular atrial fibrillation (in patients meeting at least two of: age 80 years or older, body weight 60kg or below, or serum creatinine 1.5mg/dL or above), and for the treatment and prevention of deep vein thrombosis and pulmonary embolism, and prevention of their recurrence.

MECHANISM OF ACTION

Apixaban is a highly selective, reversible, direct inhibitor of free and clot-bound factor Xa, the final common step of the intrinsic and extrinsic coagulation pathways that activates prothrombin to thrombin. By blocking factor Xa, apixaban prevents thrombin generation and subsequent fibrin clot formation without requiring antithrombin III as a cofactor, unlike heparin. The 2.5mg dose is the reduced dose used in patients with atrial fibrillation who meet the size-adjusted criteria, and as the maintenance dose for extended DVT/PE prevention after initial treatment with 5mg twice daily.

DOSAGE AND ADMINISTRATION

•      Atrial fibrillation stroke prevention (reduced dose patients): 2.5mg twice daily

•      DVT/PE prevention (extended prophylaxis): 2.5mg twice daily after initial 6 months of full-dose treatment

•      Take with or without food

•      Always follow the dosage and indication prescribed by your cardiologist or hematologist; do not switch anticoagulant regimens without specialist guidance

SIDE EFFECTS

Common:

•      Bleeding (most common; bruising, prolonged bleeding from cuts)

•      Nausea and gastrointestinal discomfort

•      Anemia

•      Elevated liver enzymes

Less common:

•      Serious or major bleeding including intracranial hemorrhage, gastrointestinal bleeding, and hematuria

•      Hypersensitivity reactions including angioedema

•      Hypotension

•      Thrombocytopenia (rare)

NOT RECOMMENDED IN

•      Active clinically significant bleeding

•      Severe hepatic impairment associated with coagulopathy

•      Lesions or conditions at significant risk of major bleeding

•      Concurrent use of other anticoagulants without specialist oversight

DRUG INTERACTIONS

Strong CYP3A4 and P-glycoprotein inhibitors such as ketoconazole, ritonavir, and clarithromycin significantly increase apixaban levels; the combination may substantially increase bleeding risk. Strong dual inducers of CYP3A4 and P-gp such as rifampicin, carbamazepine, and phenytoin reduce apixaban levels, risking inadequate anticoagulation; avoid these combinations. Aspirin and NSAIDs increase bleeding risk; use with caution. Avoid other anticoagulants and antiplatelet agents without specialist supervision.
