INDICATION

Zeegap 100mg Cap is a gabapentinoid indicated for the treatment of neuropathic pain in adults associated with diabetic peripheral neuropathy, post-herpetic neuralgia, and spinal cord injury. It is also indicated as adjunctive therapy for partial-onset seizures and for the treatment of fibromyalgia and generalized anxiety disorder.

MECHANISM OF ACTION

Pregabalin binds to the alpha-2-delta subunit of voltage-gated calcium channels in the central and peripheral nervous system. This binding reduces calcium influx into hyperexcited neurons, decreasing the release of excitatory neurotransmitters including glutamate, noradrenaline, and substance P. The result is a reduction in central sensitization and abnormal neuronal excitability that underlies neuropathic pain and partial seizures. Pregabalin has superior oral bioavailability (approximately 90%) compared to gabapentin and a more predictable dose-response relationship.

DOSAGE AND ADMINISTRATION

•      Neuropathic pain: start 75mg twice daily or 50mg three times daily; may increase to 150mg twice daily or 100mg three times daily after 1 week; maximum 300mg per day

•      Epilepsy (adjunct): 150 to 600mg per day in 2 to 3 divided doses

•      Dose reduction required in renal impairment based on creatinine clearance

•      Always follow the titration schedule prescribed by your neurologist

SIDE EFFECTS

Common:

•      Dizziness and somnolence (most common)

•      Weight gain and increased appetite

•      Peripheral oedema (swelling of extremities)

•      Dry mouth and blurred vision

Less common:

•      Cognitive impairment and concentration difficulties

•      Ataxia and incoordination

•      Withdrawal symptoms on abrupt discontinuation (reduce dose gradually)

DRUG INTERACTIONS

Oxycodone combined with pregabalin significantly increases CNS and respiratory depression risk. Angiotensin-converting enzyme inhibitors may increase oedema risk when combined with pregabalin. Pregabalin does not induce or inhibit CYP450 enzymes and has no significant pharmacokinetic drug interactions.
