INDICATION

Zavget 10mg Tab is an SSRI antidepressant indicated for the treatment of major depressive disorder (MDD) and generalized anxiety disorder (GAD) in adults and adolescents aged 12 years and older. It is also indicated for panic disorder with or without agoraphobia, social anxiety disorder, and obsessive-compulsive disorder. At 10mg, Zavget represents the standard therapeutic dose at which most patients achieve a full antidepressant response with a favorable tolerability profile, making it the recommended starting and common maintenance dose for the majority of adult patients.

MECHANISM OF ACTION

Escitalopram, the S-enantiomer of citalopram, is a highly selective serotonin reuptake inhibitor (SSRI) that blocks serotonin reuptake by binding with high affinity to the serotonin transporter (SERT). This elevates serotonin levels at postsynaptic receptors, gradually normalizing mood-related neurotransmission. Unlike citalopram, escitalopram also interacts with an allosteric SERT site, enhancing its inhibitory effect and increasing potency with a faster onset. Clinical improvements often begin within 2 weeks, with full antidepressant effects appearing over 4 to 6 weeks.

DOSAGE AND ADMINISTRATION

•      Standard adult dose: 10mg once daily, taken orally with or without food.

•      Dose may be increased to 20mg once daily after a minimum of one week if clinically required.

•      Take at the same time each day, either morning or evening.

•      Swallow whole with water; do not crush or break the tablet.

•      Do not stop abruptly; taper gradually under physician supervision to prevent discontinuation syndrome.

SIDE EFFECTS

•      Common: Nausea, dry mouth, insomnia, headache, dizziness, increased sweating, fatigue.

•      Less common: Diarrhea, tremor, palpitations, agitation, weight changes.

Zavget 10mg Tab is contraindicated with concurrent or recent (within 14 days) use of MAOIs, including phenelzine, tranylcypromine, and selegiline. It must not be co-administered with pimozide due to the QT prolongation risk. It is contraindicated in patients with known hypersensitivity to escitalopram or citalopram. Use with caution in patients with epilepsy, bipolar disorder, hepatic impairment, renal impairment, cardiac conduction abnormalities, bleeding disorders, and diabetes mellitus.

DRUG INTERACTIONS

Concurrent use with MAOIs, linezolid, lithium, tramadol, triptans, and other serotonergic drugs significantly increases serotonin syndrome risk. Co-administration with drugs that prolong the QT interval, including antiarrhythmics, antipsychotics, and certain antibiotics, requires ECG monitoring. NSAIDs and anticoagulants combined with escitalopram increase gastrointestinal and systemic bleeding risk. CYP2C19 inhibitors such as omeprazole increase escitalopram plasma concentrations.
