INDICATION

Provas-N Forte Tab is a combination analgesic and muscle relaxant indicated for the relief of moderate to severe musculoskeletal pain, including low back pain, neck pain, muscle strain and spasm, and pain associated with post-traumatic or postoperative muscle stiffness. The higher paracetamol dose provides stronger analgesic coverage compared to standard combinations.

MECHANISM OF ACTION

Paracetamol inhibits prostaglandin synthesis centrally in the CNS by inhibiting cyclooxygenase enzymes in the brain and spinal cord, reducing pain signaling and fever without significant peripheral anti-inflammatory activity. Orphenadrine citrate is a centrally acting anticholinergic agent with muscle relaxant properties that reduce skeletal muscle spasm by inhibiting acetylcholine at central and neuromuscular junction cholinergic receptors. The combination provides dual-mechanism pain relief, addressing both the pain signal (paracetamol) and the underlying muscular component (orphenadrine) of musculoskeletal conditions.

DOSAGE AND ADMINISTRATION

•      Adults: 1 tablet two to three times daily

•      Do not exceed 3 tablets in 24 hours (total paracetamol: 1950mg/day from this product)

•      Take with water; may be taken with food if stomach irritation occurs

•      Always follow the dosage prescribed by your doctor; avoid combining with other paracetamol-containing products

SIDE EFFECTS

Common:

•      Dry mouth (orphenadrine anticholinergic effect)

DRUG INTERACTIONS

Orphenadrine combined with other anticholinergic drugs, antihistamines, and tricyclic antidepressants produces additive anticholinergic side effects. Paracetamol toxicity risk increases with alcohol use and hepatotoxic medications; avoid exceeding recommended doses. CNS depressants combined with orphenadrine increase sedation. Anticoagulant effect of warfarin may be enhanced with prolonged regular paracetamol use; monitor INR.
