INDICATION

Predlone 40mg Injection contains methylprednisolone, a potent synthetic glucocorticoid indicated for a wide range of inflammatory, allergic, and autoimmune conditions. It is prescribed for severe allergic reactions, acute asthma exacerbations, rheumatoid arthritis flares, inflammatory bowel disease, systemic lupus erythematosus, nephrotic syndrome, and as adjunctive therapy in certain malignancies. The injectable form provides rapid onset of anti-inflammatory action in acute situations where oral therapy is impractical.

MECHANISM OF ACTION

Methylprednisolone exerts its anti-inflammatory and immunosuppressive effects by binding to intracellular glucocorticoid receptors. The activated receptor-steroid complex translocates to the nucleus and modulates gene transcription, upregulating anti-inflammatory proteins (lipocortin) and downregulating pro-inflammatory cytokines (IL-1, IL-6, TNF-alpha). It inhibits phospholipase A2, reducing arachidonic acid release and prostaglandin/leukotriene synthesis. Methylprednisolone also stabilizes cell membranes, reduces capillary permeability, and suppresses leukocyte migration to sites of inflammation.

DOSAGE AND ADMINISTRATION

●     Adults: 40 to 120mg intramuscularly or intravenously depending on the condition and severity

●     Intra-articular injection: 4 to 40mg depending on joint size

DRUG INTERACTIONS

CYP3A4 inducers (rifampicin, phenytoin, carbamazepine) reduce methylprednisolone levels. CYP3A4 inhibitors (ketoconazole, erythromycin) increase levels. NSAIDs combined with corticosteroids increase GI ulceration risk. Methylprednisolone may reduce the effect of antidiabetic agents and antihypertensives. Concurrent use with anticoagulants requires monitoring.
