INDICATION

Petra 20mg Caps is a proton pump inhibitor (PPI) indicated for the treatment of gastro-esophageal reflux disease (GERD), healing of erosive esophagitis, prevention of relapse of GERD, and as part of Helicobacter pylori eradication regimens. It is also used for prevention of NSAID-induced gastric ulcers in at-risk patients.

MECHANISM OF ACTION

Esomeprazole is the S-enantiomer of omeprazole and a proton pump inhibitor that selectively and irreversibly binds to the H+/K+-ATPase enzyme on the secretory surface of gastric parietal cells after acid activation. By blocking the final step of gastric acid secretion, it produces a profound and sustained suppression of both basal and stimulated acid output. As the S-isomer, esomeprazole undergoes less first-pass metabolism via CYP2C19 than racemic omeprazole, resulting in higher bioavailability and more consistent acid suppression across patient populations.

DOSAGE AND ADMINISTRATION

•      GERD and erosive esophagitis: 20mg once daily for 4 to 8 weeks

•      Maintenance of healed esophagitis: 20mg once daily

•      H. pylori eradication: 20mg twice daily with antibiotics for 7 to 14 days

•      Take 30 to 60 minutes before the first meal of the day; always follow your doctor's instructions

SIDE EFFECTS

Common:

•      Headache

•      Nausea, diarrhoea, flatulence

•      Abdominal pain

•      Dry mouth

Less common:

•      Hypomagnesaemia with prolonged use

•      Vitamin B12 deficiency with long-term therapy

•      Clostridium difficile infection risk with prolonged use

•      Fundic gland polyps (generally benign)

NOT RECOMMENDED IN

•      Known hypersensitivity to esomeprazole, omeprazole, or other PPIs

•      Concurrent use with atazanavir or nelfinavir (significantly reduced antiretroviral absorption)

•      Long-term use without regular reassessment

•      Severe hepatic impairment

DRUG INTERACTIONS

Esomeprazole inhibits CYP2C19; it has less interaction with clopidogrel than omeprazole, making it a preferred PPI in patients on antiplatelet therapy. Reduces absorption of atazanavir, nelfinavir, ketoconazole, and itraconazole. Digoxin absorption may increase. Methotrexate: PPIs may reduce renal excretion; monitor for methotrexate toxicity.
