INDICATION

Pasage 10mg Tablets contain rosuvastatin, a potent HMG-CoA reductase inhibitor (statin) indicated for the treatment of primary hypercholesterolemia (heterozygous familial and non-familial) and mixed dyslipidemia as an adjunct to diet, prevention of cardiovascular events in patients at high risk, and slowing the progression of atherosclerosis. Rosuvastatin is one of the most effective statins for lowering LDL cholesterol and is also effective at raising HDL cholesterol and lowering triglycerides.

MECHANISM OF ACTION

Rosuvastatin competitively inhibits HMG-CoA reductase, the rate-limiting enzyme in hepatic cholesterol biosynthesis that catalyzes the conversion of HMG-CoA to mevalonate. By reducing intracellular cholesterol synthesis, hepatocytes upregulate LDL receptor expression on the cell surface, increasing the clearance of LDL cholesterol from the bloodstream. Rosuvastatin is relatively hydrophilic compared to other statins, resulting in greater hepatoselectivity and less penetration into non-hepatic tissues.

DOSAGE AND ADMINISTRATION

●     Adults: start with 5 to 10mg once daily; adjust dose after 4 weeks based on lipid response

●     Maximum dose: 40mg daily (20mg in patients of Asian descent due to higher systemic exposure)

●     Take at the same time each day, with or without food

●     Swallow tablets whole with water

●     Hepatic impairment: start at 5mg; 40mg dose is contraindicated

●     Renal impairment (severe): start at 5mg; maximum 10mg daily

SIDE EFFECTS

Common:

●     Headache and dizziness

●     Myalgia (muscle pain) and muscle weakness

●     Constipation and nausea

●     Abdominal pain

Less common:

●     Rhabdomyolysis, a rare but serious condition involving severe muscle breakdown

●     Elevated liver enzymes (ALT, AST) requiring monitoring

●     New-onset type 2 diabetes, particularly at higher doses

●     Proteinuria, usually transient and not associated with renal deterioration

NOT RECOMMENDED IN

●     Known hypersensitivity to rosuvastatin or any component

●     Active liver disease or unexplained persistent elevations of serum transaminases

●     Pregnancy and breastfeeding (Category X: may cause fetal harm)

●     Concurrent use with cyclosporine

●     Patients with predisposing factors for myopathy or rhabdomyolysis

DRUG INTERACTIONS

Cyclosporine significantly increases rosuvastatin levels and is contraindicated. Gemfibrozil increases rhabdomyolysis risk and should be avoided. Warfarin INR should be monitored when initiating or adjusting rosuvastatin. Antacids containing aluminum and magnesium may reduce absorption; space by 2 hours. Protease inhibitors may increase rosuvastatin exposure.
