INDICATION

Oxa-One Syrup is an oxazolidinone antibiotic suspension indicated for the treatment of serious infections caused by susceptible gram-positive organisms in pediatric patients and adults who cannot take tablets. It is prescribed for VRE bacteremia, MRSA pneumonia, and complicated skin and soft tissue infections. The oral suspension provides accurate weight-based dosing for children.

MECHANISM OF ACTION

Linezolid binds to the 23S rRNA of the 50S ribosomal subunit, blocking the formation of the functional 70S initiation complex needed for bacterial protein synthesis. This unique target site means there is no cross-resistance with other antibiotic classes.

DOSAGE AND ADMINISTRATION

●     Adults and children over 12: 600mg (30ml) every 12 hours

●     Children 5 to 11 years: 10mg/kg every 8 hours, maximum 600mg per dose

●     Children under 5 years: 10mg/kg every 8 hours

●     Neonates under 7 days: 10mg/kg every 12 hours, increasing to every 8 hours by day 7

●     Shake well before each dose

●     Monitor CBC weekly, especially platelet count

SIDE EFFECTS

Common:

●     Diarrhea and nausea

●     Headache

●     Vomiting

●     Thrombocytopenia requiring weekly monitoring

Less common:

●     Myelosuppression with therapy exceeding 2 weeks

●     Peripheral and optic neuropathy with prolonged use

●     Lactic acidosis with extended therapy

●     Serotonin syndrome with concurrent serotonergic drugs

NOT RECOMMENDED IN

●     Known hypersensitivity to linezolid

●     Concurrent MAOI use or within 2 weeks of MAOI discontinuation

●     Concurrent serotonergic agents without careful monitoring

●     Uncontrolled hypertension or pheochromocytoma

DRUG INTERACTIONS

Linezolid is a weak MAO inhibitor. Avoid concurrent serotonergic drugs. Limit tyramine-rich foods. Sympathomimetic agents may increase blood pressure.

STORAGE INSTRUCTIONS

Store dry granules below 25 degrees Celsius. After reconstitution, store at room temperature. Use within 21 days. Do not freeze. Keep out of reach of children.
