INDICATION      

Orlifit 120mg Cap is a lipase inhibitor indicated for the management of obesity, including weight loss and weight maintenance in adult patients with a body mass index (BMI) of 30 kg/m2 or above, or 27 kg/m2 and above in the presence of weight-related comorbidities such as type 2 diabetes, hypertension, or dyslipidemia. It is used alongside a reduced-calorie diet and exercise programme.

MECHANISM OF ACTION

Orlistat inhibits gastric and pancreatic lipases, the enzymes responsible for hydrolyzing dietary triglycerides into absorbable free fatty acids and monoglycerides in the intestinal lumen. By blocking approximately one-third of dietary fat absorption, orlistat reduces caloric intake from fat. The undigested triglycerides pass through the gut and are excreted in the feces. This mechanism is entirely local, as orlistat has minimal systemic absorption and acts exclusively within the gastrointestinal tract.

DOSAGE AND ADMINISTRATION

•      Adults: 120mg three times daily, taken with each main meal containing fat or up to 1 hour after the meal

•      If a meal is very low in fat or skipped, the dose may be omitted

•      Use as part of a calorie-controlled diet and regular physical activity programme

SIDE EFFECTS

Common:

•      Oily or fatty stools (steatorrhea)

•      Fecal urgency and incontinence

Less common:

•      Deficiency of fat-soluble vitamins A, D, E, and K with prolonged use; supplement separately

•      Oxalate nephropathy (rare) with high-dose prolonged use

•      Reduced efficacy of oral contraceptives due to fat malabsorption

•      Liver injury reported rarely (post-marketing); monitor if symptoms of liver dysfunction develop

DRUG INTERACTIONS

Orlistat reduces absorption of fat-soluble vitamins; multivitamin supplement containing fat-soluble vitamins should be taken at least 2 hours before or after orlistat. Cyclosporine absorption is reduced by approximately 30% when taken with orlistat; monitor cyclosporine levels closely and take at least 3 hours apart. Anticoagulants: warfarin levels may be affected; monitor INR. Antiepileptics: valproic acid absorption may be reduced.
