INDICATION
Macdronic Injection 4mg is indicated for the prevention of skeletal-related events (pathological fractures, spinal compression, bone radiotherapy, or hypercalcemia) in patients with advanced malignancies involving bone, and for the treatment of tumor-induced hypercalcemia.
MECHANISM OF ACTION
Same mechanism as Macdronic 5mg/100ml. Zoledronic acid 4mg is the oncology dosing for bone metastases. It inhibits osteoclast-mediated bone resorption by blocking farnesyl pyrophosphate synthase. In the oncology setting, it reduces skeletal complications by strengthening bone in areas weakened by metastatic disease and reverses malignancy-associated hypercalcemia by reducing calcium release from bone.
DOSAGE AND ADMINISTRATION
● 4mg administered as a single IV infusion over no less than 15 minutes
● For bone metastases: every 3 to 4 weeks
● For hypercalcemia of malignancy: single 4mg infusion; may repeat after 7 days if calcium remains elevated
● Dilute in 100ml of 0.9% saline or 5% dextrose before infusion
● Ensure adequate hydration before and during treatment
● Adjust dose in renal impairment
SIDE EFFECTS
Common:
● Flu-like symptoms after infusion: fever, fatigue, bone pain
● Nausea
● Mild anemia
Less common:
● Renal deterioration requiring monitoring of creatinine before each dose
● Osteonecrosis of the jaw
● Hypocalcemia, sometimes severe
● Atrial fibrillation in rare cases
NOT RECOMMENDED IN
● Creatinine clearance below 35 ml/min for cancer patients
● Hypocalcemia (correct before infusion)
● Known hypersensitivity to zoledronic acid
● Pregnancy and breastfeeding
DRUG INTERACTIONS
Same as Macdronic 5mg/100ml. Aminoglycosides, loop diuretics, and nephrotoxic agents increase the risk of renal impairment and electrolyte abnormalities.
STORAGE INSTRUCTIONS
Store below 25 degrees Celsius. Protect from light. After dilution, use within 24 hours if refrigerated. Keep out of reach of children.
