INDICATION

Lezoxib 100mg Cap is a selective COX-2 inhibitor (coxib) NSAID used to relieve pain and inflammation in osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, acute pain, and primary dysmenorrhea. It is preferred in patients with gastrointestinal risk factors, as it has a significantly lower risk of gastric ulceration compared to non-selective NSAIDs.

MECHANISM OF ACTION

Celecoxib selectively inhibits cyclooxygenase-2 (COX-2), the isoenzyme predominantly induced at sites of inflammation and responsible for the synthesis of pro-inflammatory prostaglandins. By targeting COX-2 while sparing COX-1 (which protects the gastric mucosa and maintains platelet function), celecoxib reduces inflammation, pain, and fever with a significantly lower risk of gastrointestinal side effects than non-selective NSAIDs such as ibuprofen or diclofenac.

DOSAGE AND ADMINISTRATION

•      Osteoarthritis: 100mg twice daily or 200mg once daily

•      Rheumatoid arthritis: 100 to 200mg twice daily

•      Acute pain and dysmenorrhea: 400mg on day 1, then 200mg twice daily as needed

•      Always use the lowest effective dose for the shortest duration; follow your doctor's instructions

SIDE EFFECTS

Common:

•      Headache

•      Abdominal pain, dyspepsia

•      Diarrhoea

•      Peripheral oedema (fluid retention)

Less common:

•      Increased cardiovascular risk (heart attack, stroke) with prolonged high-dose use

•      Elevated blood pressure

•      Elevated liver enzymes or hepatotoxicity

•      Serious skin reactions (Stevens-Johnson syndrome, rare)

DRUG INTERACTIONS

Celecoxib inhibits CYP2D6 and may increase levels of drugs metabolized by this enzyme, including tricyclic antidepressants and some antipsychotics. Fluconazole inhibits celecoxib metabolism and can double its plasma levels; reduce dose if co-prescribed. Warfarin anticoagulation may be enhanced, particularly in elderly patients; monitor INR. ACE inhibitors and diuretics may have reduced efficacy due to prostaglandin inhibition.
