INDICATION

Foster NEXThaler 100/6 is indicated for the regular maintenance treatment of asthma in adults where a combination inhaled corticosteroid and long-acting beta-2 agonist is appropriate. It is suitable for patients not adequately controlled on ICS monotherapy who require step-up therapy.

MECHANISM OF ACTION

The NEXThaler device delivers extra fine particles of beclomethasone dipropionate and formoterol fumarate as a dry powder. Beclomethasone reduces airway inflammation through anti-inflammatory gene modulation and reduction of eosinophil activity. Formoterol provides long-acting bronchodilation by relaxing airway smooth muscle via beta-2 receptor stimulation. The extra fine particle technology ensures uniform drug deposition in both large and small airways, improving overall lung function compared to non-extra fine formulations. The breath-actuated mechanism of NEXThaler ensures reliable drug delivery with minimal coordination effort.

DOSAGE AND ADMINISTRATION

•      Adults: 1 to 2 inhalations twice daily

•      Maximum: 2 inhalations twice daily

•      Inhale deeply and steadily through the mouthpiece; the device is breath-actuated

•      Do not exhale into the device

•      Rinse mouth after inhalation to reduce thrush risk

•      Do not use as a rescue inhaler for acute attacks

SIDE EFFECTS

Common:

•      Oral thrush (oropharyngeal candidiasis)

•      Mild headache

•      Tremor or palpitations

Less common:

•      Voice changes or hoarseness

•      Paradoxical bronchospasm

•      Systemic steroid effects with high-dose prolonged use

DRUG INTERACTIONS

Beta-blockers may reduce the bronchodilator effect. Other sympathomimetics may increase cardiovascular side effects. CYP3A4 inhibitors may increase systemic corticosteroid exposure. QTc-prolonging drugs require caution with concurrent formoterol.
