INDICATION

Fiberon Sachet is indicated for the prevention and treatment of acute diarrhea including antibiotic-associated diarrhea, traveler's diarrhea, and diarrhea due to Clostridium difficile. It is also used as an adjunct in the management of inflammatory bowel syndrome, irritable bowel syndrome, and restoration of gut flora after gastrointestinal infections.

MECHANISM OF ACTION

Saccharomyces boulardii is a non-pathogenic yeast probiotic that exerts its effect through multiple mechanisms. It produces proteases that cleave bacterial toxin receptors on intestinal epithelial cells, preventing toxin binding. It secretes zinc-dependent proteases that degrade Clostridium difficile toxins A and B. It increases secretory IgA levels to enhance mucosal immunity. It competes with pathogens for adhesion sites on the gut epithelium and produces short-chain fatty acids that support colonocyte integrity. Unlike bacterial probiotics, Saccharomyces boulardii is intrinsically resistant to antibiotics, allowing concurrent use during antibiotic therapy.

DOSAGE AND ADMINISTRATION

·        Adults: 1 sachet (250mg to 500mg) twice daily.

·        Children above 2 years: 1 sachet once or twice daily or as prescribed. Dissolve the sachet contents in water, juice, or food.

·        Do not dissolve in hot liquids above 50 degrees Celsius as heat destroys the live yeast cells.

·        Best taken with meals.

·        Continue for the duration prescribed, typically 5 to 10 days for acute diarrhea.

SIDE EFFECTS

Common:

•      Bloating or flatulence in the first few days of use

•      Mild nausea in sensitive individuals

Less common:

•      Fungemia in severely immunocompromised patients in rare cases

•      Allergic reactions in patients with yeast hypersensitivity

NOT RECOMMENDED IN

•      Severely immunocompromised patients including bone marrow transplant recipients and patients with central venous catheters

•      Known hypersensitivity to yeast or Saccharomyces species

•      Patients with yeast-related conditions such as Candida overgrowth should use with caution

DRUG INTERACTIONS

•      Antifungal agents such as fluconazole may reduce the efficacy of Saccharomyces boulardii

•      Unlike bacterial probiotics, it is not destroyed by antibiotics

STORAGE INSTRUCTIONS

Store in a cool and dry place below 25 degrees Celsius. Protect from moisture and direct heat. Do not expose to temperatures above 50 degrees Celsius. Keep out of reach of children.
