INDICATION

Etipro 20mg Cap is a proton pump inhibitor used for the treatment of GERD, duodenal and gastric ulcers, H. pylori eradication in combination therapy, and prevention of NSAID-induced gastric ulcers in high-risk patients. It is also used for the long-term management of Zollinger-Ellison syndrome.

MECHANISM OF ACTION

Omeprazole is a prodrug activated in the acidic gastric canaliculi of parietal cells. The active form irreversibly binds to the H+/K+-ATPase proton pump, blocking the final step of gastric acid secretion. This produces a dose-dependent reduction in both basal and stimulated acid output, raising intragastric pH and allowing mucosal healing.

DOSAGE AND ADMINISTRATION

•      GERD: 20mg once daily for 4 weeks; extend to 8 weeks if needed

•      Duodenal ulcer: 20mg once daily for 2 to 4 weeks

•      H. pylori eradication: 20mg twice daily with antibiotics for 7 to 14 days

•      Take 30 to 60 minutes before the first meal

•      Swallow capsule whole; may open and sprinkle granules on acidic food for patients who cannot swallow

SIDE EFFECTS

Common:

•      Headache, nausea

•      Diarrhoea, abdominal pain

•      Constipation, flatulence

Less common:

•      Hypomagnesaemia with prolonged use

•      Vitamin B12 deficiency

•      C. difficile diarrhoea

•      Osteoporosis-related fractures

NOT RECOMMENDED IN

•      Hypersensitivity to omeprazole or other PPIs

•      Concurrent nelfinavir use

•      Caution in severe hepatic impairment

DRUG INTERACTIONS

Reduces absorption of pH-dependent drugs. Inhibits CYP2C19 — increases diazepam, phenytoin, warfarin levels. May reduce clopidogrel efficacy.
