INDICATION

Eptrim-X 80/480mg Tab is an artemisinin-based combination therapy (ACT) used for the treatment of uncomplicated Plasmodium falciparum malaria in adults and adolescents weighing 35kg or more. WHO recommends ACTs as first-line treatment for uncomplicated falciparum malaria due to their high cure rates and rapid parasite clearance. This combination addresses both the rapid-acting and long-acting phases of antimalarial treatment.

MECHANISM OF ACTION

Artemether rapidly kills malaria parasites by generating reactive oxygen species inside the parasite's food vacuole. Once activated by iron from haem digestion, artemether produces free radicals that alkylate key parasite proteins and damage cellular membranes, causing rapid parasite death across all erythrocytic stages. Lumefantrine is a long-acting antimalarial that inhibits the conversion of haem to hemozoin in the parasite food vacuole, leading to toxic haem accumulation and oxidative damage to the parasite. The combination targets two different mechanisms: artemether provides rapid parasite clearance in the first 48 hours, while lumefantrine eliminates residual parasites over the following days, reducing the risk of recrudescence and resistance development.

DOSAGE AND ADMINISTRATION

•      Adults and adolescents (above 35kg): 4 tablets at time 0, then 4 tablets at 8 hours, then 4 tablets twice daily for the following 2 days (total 24 tablets over 3 days)

•      Take with food or a fatty drink to maximize lumefantrine absorption

•      Do not take on an empty stomach as lumefantrine absorption is severely reduced without fat

•      Complete the full 3-day course even if symptoms improve after the first dose

•      If vomiting occurs within 1 hour of dosing, repeat the dose

SIDE EFFECTS

Common:

•      Headache, dizziness

•      Nausea, vomiting, abdominal pain

•      Sleep disturbances

•      Anorexia, fatigue

Less common:

•      QT prolongation (monitor ECG in patients with cardiac risk factors or on other QT-prolonging drugs)

•      Palpitations, tachycardia

•      Hemolysis in G6PD-deficient patients (delayed onset, 2 to 4 weeks after treatment)

•      Elevated liver enzymes

NOT RECOMMENDED IN

•      Known hypersensitivity to artemether, lumefantrine, or any excipient

•      History of cardiac arrhythmia, clinically relevant bradycardia, or known QTc prolongation

•      First trimester of pregnancy (use alternative antimalarial if possible)

•      Concurrent use with other QT-prolonging drugs (halofantrine, antipsychotics, class IA/III antiarrhythmics)

•      Severe malaria (IV artesunate is the treatment of choice for severe malaria)

DRUG INTERACTIONS

Strong CYP3A4 inhibitors (ketoconazole, ritonavir, grapefruit juice) increase lumefantrine plasma levels, raising the risk of QT prolongation. Strong CYP3A4 inducers (rifampicin, efavirenz, carbamazepine, phenytoin) reduce lumefantrine exposure, potentially reducing antimalarial efficacy. Halofantrine and mefloquine should not be used within 4 weeks of artemether-lumefantrine treatment due to additive cardiac effects. QT-prolonging drugs (antipsychotics, fluoroquinolones, macrolide antibiotics) increase the risk of ventricular arrhythmia when combined.
