INDICATION

Epan 100mg Syrup is a cephalosporin antibiotic indicated for the treatment of mild to moderate bacterial infections in children including otitis media, pharyngitis, tonsillitis, urinary tract infections, uncomplicated gonorrhea, and respiratory tract infections caused by susceptible organisms.

MECHANISM OF ACTION

Cefixime is a third-generation oral cephalosporin that exerts bactericidal activity by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) on the bacterial cell membrane, preventing cross-linking of peptidoglycan chains, which leads to cell lysis and death. It is effective against a broad spectrum of gram-positive and gram-negative organisms.

DOSAGE AND ADMINISTRATION

·        Children: 8mg/kg/day in a single dose or divided into two doses.

·        Standard dosing: 5ml (100mg) once daily or 2.5ml (50mg) twice daily. Shake the bottle well before each use.

·        Administer with or without food.

·        Complete the full prescribed course even if symptoms resolve earlier.

SIDE EFFECTS

Common:

•      Diarrhea or loose stools

•      Nausea or mild stomach upset

•      Abdominal pain

Less common:

•      Allergic reactions including skin rash or urticaria

•      Headache or dizziness

•      Elevated liver enzymes

NOT RECOMMENDED IN

•      Patients with hypersensitivity to cefixime, other cephalosporins, or beta-lactam antibiotics

•      Patients with a history of antibiotic-associated colitis

•      Use with caution in patients with renal impairment

DRUG INTERACTIONS

•      Carbamazepine levels may increase when co-administered with cefixime

•      Anticoagulants such as warfarin may have enhanced effects

•      Probenecid may reduce renal clearance of cefixime

STORAGE INSTRUCTIONS

Store in a cool and dry place below 25 degrees Celsius. After reconstitution, store in the refrigerator and use within 14 days. Shake well before each use. Keep out of reach of children.
