INDICATION

Combivair 400mcg/6mcg Cap is a combination inhaled corticosteroid and long-acting beta-2 agonist (LABA) for inhalation, used in the maintenance treatment of asthma and chronic obstructive pulmonary disease (COPD). It is prescribed for patients whose symptoms are not adequately controlled with an inhaled corticosteroid alone and who require a bronchodilator as regular treatment.

MECHANISM OF ACTION

Budesonide is an inhaled corticosteroid that binds to glucocorticoid receptors in airway cells, reducing the production of inflammatory mediators including cytokines, prostaglandins, and histamine. Formoterol is a selective beta-2 adrenoceptor agonist that relaxes bronchial smooth muscle rapidly and sustains bronchodilation for up to 12 hours. Together, the two components provide superior asthma control compared to either agent alone.

DOSAGE AND ADMINISTRATION

•      Adults and adolescents above 12 years: 1 capsule inhaled twice daily via dry powder inhaler

•      Do not swallow the capsule; for inhalation only

•      Rinse mouth with water after each inhalation to prevent oral candidiasis

•      Do not increase the dose without medical advice

•      Carry a short-acting reliever inhaler for acute attacks

SIDE EFFECTS

Common:

•      Oral candidiasis (thrush)

•      Hoarseness or voice changes

•      Headache, palpitations

•      Tremor, especially of the hands

Less common:

•      Adrenal suppression with prolonged high-dose use

•      Reduced bone mineral density with long-term therapy

•      Hypokalemia (low potassium), especially at high doses

•      Paradoxical bronchospasm (stop use if this occurs)

NOT RECOMMENDED IN

•      Not for use in acute asthma attacks or acute bronchospasm

•      Known hypersensitivity to budesonide, formoterol, or lactose

•      Should not be used in children under 12 years

•      Use with caution in patients with tuberculosis, severe cardiovascular disease, or diabetes

DRUG INTERACTIONS

Strong CYP3A4 inhibitors such as ketoconazole, itraconazole, and ritonavir can significantly increase systemic budesonide exposure. Beta blockers (including ophthalmic preparations) can reduce the bronchodilator effect of formoterol. QT-prolonging drugs combined with formoterol increase the risk of arrhythmias.

Store Combivair 400mcg capsules at room temperature below 25°C. Protect from moisture and heat. Keep capsules in original packaging and only remove immediately before use. Store out of reach of children.
