INDICATION

Cadlizo 600mg Tab is an oxazolidinone antibiotic indicated for the treatment of serious and life-threatening gram-positive bacterial infections caused by susceptible organisms including vancomycin-resistant Enterococcus faecium (VRE) infections, nosocomial pneumonia caused by methicillin-resistant Staphylococcus aureus (MRSA), community-acquired pneumonia caused by susceptible gram-positive pathogens, and complicated skin and soft tissue infections. It is a reserve antibiotic used when other agents have failed or are contraindicated.

MECHANISM OF ACTION

Linezolid is the first member of the oxazolidinone class of synthetic antibiotics. It inhibits bacterial protein synthesis through a unique mechanism that is distinct from all other antibiotic classes: it binds to the 23S ribosomal RNA of the 50S ribosomal subunit at a site that prevents the formation of the 70S ribosomal initiation complex, which is the first step in the translation process. Because this binding site is unique, there is no cross-resistance with other antibiotic classes including macrolides, aminoglycosides, fluoroquinolones, or glycopeptides, making it effective against many multidrug-resistant gram-positive organisms. Linezolid also acts as a reversible, non-selective monoamine oxidase (MAO) inhibitor, which is responsible for some of its drug and food interactions.

DOSAGE AND ADMINISTRATION

•      Adults: 600mg orally every 12 hours for 10 to 28 days depending on indication

•      May be taken with or without food; take at the same time each day

•      Avoid tyramine-rich foods during treatment (see drug interactions)

•      Always complete the full prescribed course under specialist supervision; weekly CBC monitoring is mandatory for courses exceeding 2 weeks

SIDE EFFECTS

Common:

•      Nausea, vomiting, and diarrhoea

•      Headache

•      Taste disturbances and oral discoloration (tongue discoloration)

•      Elevated liver enzymes

Less common:

•      Myelosuppression: anemia, thrombocytopenia, and neutropenia (monitor CBC weekly)

•      Peripheral and optic neuropathy with prolonged therapy beyond 28 days

•      Serotonin syndrome when combined with serotonergic drugs (see interactions)

•      Lactic acidosis (rare but serious)

NOT RECOMMENDED IN

•      Known hypersensitivity to linezolid or any oxazolidinone

•      Concurrent use with MAO inhibitors or within 2 weeks of stopping them

•      Uncontrolled hypertension, phaeochromocytoma, or carcinoid syndrome without monitoring

•      Pregnancy and breastfeeding unless benefit clearly outweighs risk

DRUG INTERACTIONS

Linezolid acts as a reversible non-selective MAO inhibitor. Concurrent SSRIs, SNRIs, tricyclic antidepressants, triptans, tramadol, and other serotonergic drugs significantly increase the risk of serotonin syndrome; these combinations are generally contraindicated. Tyramine-rich foods including aged cheeses, fermented meats, smoked fish, broad bean pods, and tap beer must be avoided during treatment to prevent hypertensive crisis. Adrenergic agents (sympathomimetics, vasopressors) produce exaggerated pressor responses. Rifampicin reduces linezolid plasma levels through enzyme induction.
