INDICATION

Brimo 0.2% Drop is a selective alpha-2 adrenergic receptor agonist ophthalmic solution indicated for the reduction of elevated intraocular pressure (IOP) in patients with open-angle glaucoma or ocular hypertension, as monotherapy or as adjunctive therapy when IOP is inadequately controlled with a single agent. It is also used for short-term IOP reduction before and after laser eye procedures.

MECHANISM OF ACTION

Brimonidine is a selective alpha-2 adrenergic agonist that lowers intraocular pressure through two complementary mechanisms. It reduces aqueous humour production by stimulating presynaptic alpha-2 receptors in the ciliary body, decreasing cAMP levels and reducing the rate of aqueous secretion. It also increases uveoscleral outflow by relaxing the longitudinal ciliary muscle and dilating the episcleral and uveal vessels. The combination of reduced production and increased outflow results in a clinically meaningful reduction in IOP. Unlike beta-blockers, brimonidine has no cardiovascular or pulmonary effects, making it safer in patients with asthma and cardiovascular disease.

DOSAGE AND ADMINISTRATION

•      Instil 1 drop into the affected eye(s) three times daily, approximately 8 hours apart

•      Remove contact lenses before instillation; wait at least 15 minutes before reinserting

•      Do not touch the dropper tip to any surface to prevent contamination

•      Always follow the dosing schedule prescribed by your ophthalmologist; regular IOP monitoring is essential

SIDE EFFECTS

Common:

•      Ocular hyperemia (redness) and burning on instillation

•      Follicular conjunctivitis (common adverse effect; may require discontinuation)

•      Ocular allergy and conjunctival blanching

•      Dry mouth and dry eyes (systemic alpha-2 effect through nasolacrimal absorption)

Less common:

•      Drowsiness, fatigue, and somnolence (significant systemic absorption through nasolacrimal drainage)

•      Headache

•      Hypotension and bradycardia (systemic alpha-2 effect)

•      Corneal epitheliopathy with prolonged use

NOT RECOMMENDED IN

•      Concurrent use with MAO inhibitors (systemic hypertensive crisis risk from CNS amine interactions)

•      Infants and children under 2 years (risk of life-threatening CNS and respiratory depression)

•      Severe cardiovascular disease, including orthostatic hypotension

•      Known hypersensitivity to brimonidine or any excipient

DRUG INTERACTIONS

MAO inhibitors are contraindicated with brimonidine due to systemic interaction with alpha-2 agonist effects. CNS depressants including alcohol, barbiturates, and sedatives have additive CNS depression effects through systemic absorption. Antihypertensives may have additive hypotensive effects. Tricyclic antidepressants may blunt brimonidine's IOP-lowering efficacy. Beta-blockers combined with brimonidine have additive IOP-lowering effects and may be used in combination therapy.
