INDICATION

Bisleri-S IV Injection 100mg/5ml is an intravenous iron preparation indicated for the treatment of iron-deficiency anemia in patients where oral iron therapy is insufficient, not tolerated, or not feasible. It is particularly indicated in patients with chronic kidney disease on hemodialysis or peritoneal dialysis, patients with inflammatory bowel disease, and in severe iron deficiency anemia during pregnancy when oral iron fails to achieve adequate response. It replenishes iron stores more rapidly than oral iron.

MECHANISM OF ACTION

Iron sucrose is a complex of polynuclear iron (III)-hydroxide in sucrose that is administered intravenously. After injection, the iron sucrose complex is taken up by the reticuloendothelial system, predominantly in the liver and spleen, where it is dissociated into iron and sucrose. The released iron is transported via transferrin to bone marrow erythroid precursors where it is incorporated into hemoglobin. Iron sucrose has a lower risk of anaphylaxis compared to older intravenous iron preparations (iron dextran) and provides rapid and efficient iron delivery to tissues, bypassing the gastrointestinal absorption limitations of oral iron.

DOSAGE AND ADMINISTRATION

•      Adults: 100 to 200mg per infusion session, administered 2 to 3 times per week, with total dose calculated based on hemoglobin deficit and body weight

•      Administer by slow intravenous infusion diluted in normal saline over 15 to 30 minutes, or as a slow IV injection

•      Pre-dose test dose is not required for iron sucrose (unlike iron dextran)

•      All doses prescribed and monitored by a physician; monitor hematological parameters during therapy

SIDE EFFECTS

Common:

•      Hypotension (most common; infuse slowly and monitor blood pressure)

•      Nausea and vomiting

•      Cramps and diarrhoea

•      Headache and dizziness

Less common:

•      Hypersensitivity reactions including anaphylactoid reactions (rare but have resuscitation equipment available)

•      Chest pain and dyspnea (discontinue infusion if these occur)

•      Elevated liver enzymes

•      Iron overload with excessive cumulative dosing (monitor serum ferritin)

NOT RECOMMENDED IN

•      Iron overload conditions such as haemochromatosis and hemosiderosis

•      Anemia not caused by iron deficiency

•      Known hypersensitivity to iron sucrose or any excipient

•      First trimester of pregnancy without clear medical necessity

DRUG INTERACTIONS

Oral iron supplements should not be initiated within 5 days of intravenous iron sucrose administration as they may reduce absorption. ACE inhibitors combined with intravenous iron may increase the risk of hypersensitivity-type reactions. Avoid mixing iron sucrose with other medications in the same infusion bag as compatibility data are limited.

STORAGE INSTRUCTIONS

Store between 2°C and 25°C. Do not freeze. Do not dilute in solutions other than normal saline. Use diluted solution immediately. Keep out of reach of children.
