INDICATION

Baromalt 500mg Inj is an intravenous iron preparation used to treat iron deficiency anemia when oral iron therapy is ineffective, not tolerated, or clinically inappropriate. It is indicated in patients with inflammatory bowel disease, chronic kidney disease, postpartum iron deficiency, preoperative anemia, and patients receiving erythropoiesis-stimulating agents who require rapid iron repletion.

MECHANISM OF ACTION

Ferric carboxymaltose is a macromolecular complex of ferric hydroxide and carboxymaltose. This stable complex resists release of iron in free ionic form in the bloodstream, delivering iron in a controlled manner to the reticuloendothelial system. Iron is then transferred to transferrin and ferritin, the body's iron-binding and storage proteins. From these stores, iron is made available to the bone marrow for hemoglobin synthesis and red blood cell production.

DOSAGE AND ADMINISTRATION

•      Dose calculated based on patient weight and hemoglobin level using the Ganzoni formula

•      Maximum single dose: 15mg/kg body weight, up to 1000mg elemental iron

•      Administer by slow IV infusion over at least 15 minutes per 500mg dose

•      Doses may be repeated at intervals of at least 7 days

•      Only administer under medical supervision with facilities for managing hypersensitivity

SIDE EFFECTS

Common:

•      Nausea, headache, dizziness

•      Injection site reactions: pain, redness

•      Hypophosphatemia (low phosphate), especially with repeated dosing

•      Flushing, hypertension

Less common:

•      Hypersensitivity reactions including anaphylaxis

•      Transient hypertension or hypotension

•      Arthralgia and back pain

•      Skin discoloration at injection site if extravasation occurs

NOT RECOMMENDED IN

•      Known hypersensitivity to ferric carboxymaltose or any component

•      Iron overload states such as haemochromatosis or hemosiderosis

•      Anemia not caused by iron deficiency

•      First trimester of pregnancy

DRUG INTERACTIONS

Ferric carboxymaltose should not be mixed with other medications or added to other parenteral nutrition solutions. Oral iron preparations should not be given within 5 days of IV iron therapy as oral iron absorption is suppressed. ACE inhibitors used concomitantly with parenteral iron preparations may increase the risk of systemic reactions including facial flushing, hypotension, and dyspnea.

STORAGE INSTRUCTIONS

Store Baromalt 500mg Inj at room temperature below 30°C. Do not refrigerate or freeze. Protect from light. Once opened, use immediately and discard any unused portion. Keep out of reach of children.
