INDICATION

Azobar 250mg is a macrolide antibiotic indicated for the treatment of mild to moderate bacterial infections including community-acquired pneumonia, pharyngitis and tonsillitis caused by Streptococcus pyogenes (when the patient cannot tolerate first-line penicillin therapy), acute exacerbations of chronic bronchitis, acute sinusitis, skin and soft tissue infections, and urogenital infections caused by Chlamydia trachomatis. The 250mg dose is used in standard 5-day treatment courses for respiratory and soft tissue infections.

MECHANISM OF ACTION

Azithromycin binds reversibly to the 50S ribosomal subunit of susceptible bacteria, blocking peptidyl translocation and inhibiting RNA-dependent protein synthesis. It is bacteriostatic at standard concentrations and bactericidal at high concentrations against specific organisms. Its unique pharmacokinetic profile includes extensive tissue distribution, very high intracellular concentrations in phagocytes and macrophages (which are delivered to infection sites), and a tissue half-life of 68 hours, allowing short 3 to 5-day courses to provide up to 10 days of effective antimicrobial coverage.

DOSAGE AND ADMINISTRATION

•      Standard 5-day z-pack: 500mg (two 250mg tablets) on day 1, then 250mg (one tablet) on days 2 to 5

•      Chlamydia urogenital infections: 1g (four tablets) as a single dose

•      Take with or without food

•      Always complete the full course as prescribed by your doctor

SIDE EFFECTS

Common:

•      Nausea, vomiting, and diarrhoea

•      Abdominal pain and dyspepsia

•      Headache

•      Dizziness

Less common:

•      QT prolongation and cardiac arrhythmias (risk in patients with pre-existing cardiac disease)

•      Clostridium difficile-associated diarrhoea

•      Cholestatic jaundice and elevated liver enzymes

•      Hypersensitivity reactions including rash and angioedema

NOT RECOMMENDED IN

•      Known hypersensitivity to azithromycin, erythromycin, or other macrolide antibiotics

•      History of cholestatic jaundice or hepatic dysfunction with prior azithromycin use

•      Patients with QT prolongation risk or on QT-prolonging drugs

•      Severe hepatic impairment

DRUG INTERACTIONS

Azithromycin prolongs the QT interval; avoid concurrent use with other QT-prolonging agents. Antacids containing aluminium or magnesium reduce absorption rate; take azithromycin 1 hour before or 2 hours after antacids. Warfarin anticoagulant effect may be enhanced; monitor INR. Cyclosporine levels may increase; monitor closely.
