INDICATION

Amkay 500mg Inj is the highest-strength amikacin ampoule indicated for the treatment of severe systemic gram-negative bacterial infections in adults, particularly those in intensive care settings with nosocomial pneumonia, septicemia, peritonitis, and multi-drug-resistant gram-negative infections where aminoglycoside therapy is required.

MECHANISM OF ACTION

Same mechanism as Amkay 100mg and 250mg. At 500mg, this formulation provides the concentrated amikacin dose typically needed for full adult daily dosing in a single or twice-daily regimen, reducing the number of ampoules required and facilitating once-daily high-dose aminoglycoside regimens, which are increasingly used to maximize bactericidal concentration-dependent killing while minimizing toxicity.

DOSAGE AND ADMINISTRATION

•      Adults: 15mg/kg/day; for once-daily dosing (extended-interval regimen), the full daily dose may be given as a single 500mg infusion

•      Once-daily aminoglycoside regimens provide equivalent efficacy with potentially less nephrotoxicity; use with appropriate monitoring

•      Administer by slow IV infusion over 30 to 60 minutes; not by rapid bolus injection

•      Therapeutic drug monitoring is mandatory; always prescribed and monitored by an infectious disease specialist or intensivist

SIDE EFFECTS

Common:

•      Nephrotoxicity

Less common:

•      Irreversible hearing loss

DRUG INTERACTIONS

Same as Amkay 100mg and 250mg. All nephrotoxic combinations, loop diuretics, and neuromuscular blockers carry the same interaction risks. Physical incompatibility in the same syringe with penicillins and cephalosporins.
