INDICATION

Amkay 250mg Inj is an aminoglycoside antibiotic indicated for the treatment of serious gram-negative bacterial infections including those caused by gentamicin and tobramycin-resistant strains of Pseudomonas aeruginosa, Klebsiella, Enterobacter, Serratia, Acinetobacter, and Escherichia coli. This strength is suitable for adult dosing where multiple ampoules of the lower strength would otherwise be needed.

MECHANISM OF ACTION

Same mechanism as Amkay 100mg. Amikacin irreversibly binds to the 30S ribosomal subunit causing mRNA misreading and production of non-functional proteins, leading to bacterial cell death. The 250mg strength provides a higher unit dose concentration for adult patients requiring systemic aminoglycoside therapy.

DOSAGE AND ADMINISTRATION

•      Adults: 15mg/kg/day in 2 to 3 divided doses; a 250mg ampoule represents a partial dose for a standard adult

•      Administer by slow intravenous infusion over 30 to 60 minutes or by deep intramuscular injection

•      Therapeutic drug monitoring of peak (25 to 35 mcg/ml) and trough (below 10 mcg/ml) levels mandatory

•      Dose reduction and interval adjustment required in renal impairment; always prescribed and monitored by a specialist

SIDE EFFECTS

Common:

•      Nephrotoxicity (monitor creatinine and urine output daily)

•      Ototoxicity: auditory and vestibular toxicity

•      Injection site pain

•      Nausea with rapid IV infusion

Less common:

•      Irreversible hearing loss

•      Acute tubular necrosis

•      Hypersensitivity reactions

•      Neuromuscular blockade at high levels

NOT RECOMMENDED IN

•      Known hypersensitivity to aminoglycosides

•      Pre-existing severe renal impairment without monitoring

•      Concurrent ototoxic and nephrotoxic agents without intensive monitoring

•      Myasthenia gravis

DRUG INTERACTIONS

Same as Amkay 100mg. Nephrotoxic agents, loop diuretics, neuromuscular blocking agents, and NSAIDs all interact adversely. Physical incompatibility with penicillins, cephalosporins, and heparin in the same syringe.
