INDICATION

Amkay 100mg Inj is an aminoglycoside antibiotic indicated for the treatment of serious and life-threatening bacterial infections caused by susceptible gram-negative organisms including Pseudomonas aeruginosa, Escherichia coli, Klebsiella, Enterobacter, Serratia, Acinetobacter, and Staphylococcus aureus. It is particularly valuable in infections caused by organisms resistant to gentamicin and tobramycin. It is used in nosocomial pneumonia, urinary tract infections, septicemia, bacterial endocarditis, peritonitis, and bone and joint infections.

MECHANISM OF ACTION

Amikacin is a semi-synthetic aminoglycoside antibiotic derived from kanamycin A. It irreversibly binds to the 30S ribosomal subunit of susceptible bacteria, interfering with mRNA translation and causing misreading of the genetic code, resulting in production of non-functional proteins and bactericidal death. Its unique structural features make it resistant to most aminoglycoside-inactivating enzymes produced by resistant bacteria, retaining activity against many gentamicin and tobramycin-resistant gram-negative organisms. It is particularly valuable as a component of combination therapy for severe gram-negative infections.

DOSAGE AND ADMINISTRATION

•      Adults: 15mg/kg/day in 2 or 3 divided doses by IM or IV injection

•      Children: 15 to 22.5mg/kg/day in 2 to 3 divided doses

•      Neonates: 7.5 to 10mg/kg every 12 to 24 hours based on gestational and postnatal age

•      Therapeutic drug monitoring (peak and trough levels) is mandatory for safe and effective dosing; dose adjustment required in renal impairment

SIDE EFFECTS

Common:

•      Nephrotoxicity: elevated creatinine and reduced urine output (monitor renal function daily)

•      Ototoxicity: cochlear (hearing loss and tinnitus) and vestibular (dizziness, vertigo) damage

•      Injection site pain with intramuscular administration

•      Neuromuscular blockade at high doses

Less common:

•      Irreversible hearing loss with prolonged use or excessive plasma levels

•      Renal tubular necrosis with severe nephrotoxicity

•      Hypersensitivity reactions including rash

•      Hypomagnesaemia and hypokalaemia with prolonged therapy

NOT RECOMMENDED IN

•      Known hypersensitivity to amikacin or other aminoglycosides (cross-sensitivity possible)

•      Pre-existing severe renal impairment without dose adjustment and monitoring

•      Concurrent use with other nephrotoxic or ototoxic agents without mandatory monitoring

•      Myasthenia gravis (may exacerbate neuromuscular weakness)

DRUG INTERACTIONS

Other nephrotoxic agents including amphotericin B, vancomycin, and ciclosporin significantly increase nephrotoxicity risk; avoid or monitor intensively. Furosemide and other loop diuretics increase ototoxicity risk; avoid concurrent use. Neuromuscular blocking agents have potentiated effects with aminoglycosides; use with great caution in anaesthesia. NSAIDs reduce renal elimination of aminoglycosides. Cephalosporins are physically incompatible with aminoglycosides in the same syringe.
