INDICATION

Alofast 120mg Tab is a second-generation antihistamine used to relieve the symptoms of seasonal allergic rhinitis (hay fever) and chronic idiopathic urticaria (hives). It effectively reduces sneezing, runny and itchy nose, watery and red eyes, skin itching, and hives. Because it does not cross the blood-brain barrier in significant amounts, it provides allergy relief without causing the drowsiness commonly seen with older antihistamines.

MECHANISM OF ACTION

Fexofenadine selectively and competitively blocks peripheral H1 histamine receptors. When allergens trigger the release of histamine in the body, fexofenadine prevents histamine from binding to H1 receptors on blood vessels, mucous membranes, and sensory nerve endings. This blocks the cascade of allergic symptoms including vasodilation, increased vascular permeability, itching, and excessive mucus secretion. Because fexofenadine has poor central nervous system penetration, it achieves this relief without sedation.

DOSAGE AND ADMINISTRATION

•      Adults and children over 12 years: 120mg once daily for allergic rhinitis

•      Chronic urticaria: 180mg once daily may be preferred (consult a doctor)

•      Patients with renal impairment: starting dose of 60mg once daily

•      Take with water; avoid taking with fruit juices such as grapefruit, orange, or apple juice as they may reduce absorption

•      Can be taken with or without food

SIDE EFFECTS

Common:

•      Headache

•      Nausea and indigestion

•      Dizziness (uncommon but possible)

•      Drowsiness (rare at standard doses)

Less common:

•      Viral infections such as common cold or flu-like symptoms

•      Back pain and painful menstrual periods

•      Hypersensitivity reactions including rash, pruritus, urticaria, or angioedema

•      Cardiac arrhythmia has been reported rarely at very high doses

•      Known hypersensitivity to fexofenadine or any ingredient in the formulation

•      Not recommended in children under 12 years of age for 120mg dose

•      Use with caution in patients with severe kidney disease

DRUG INTERACTIONS

Erythromycin and ketoconazole can increase plasma concentrations of fexofenadine by inhibiting its intestinal transport, which may enhance or prolong its effects. Antacids containing aluminium or magnesium hydroxide can reduce fexofenadine absorption and should be taken at least 2 hours apart. Fruit juices, particularly grapefruit, orange, and apple juice, significantly reduce the oral bioavailability of fexofenadine and should be avoided around the time of dosing. Fexofenadine does not undergo hepatic metabolism and therefore has minimal interaction with drugs metabolised by cytochrome P450 enzymes.
