INDICATION

Aine 100mg Tab is used to treat a range of mental health conditions including major depressive disorder (MDD), obsessive-compulsive disorder (OCD), panic disorder, post-traumatic stress disorder (PTSD), social anxiety disorder, and premenstrual dysphoric disorder (PMDD). It is one of the most widely prescribed antidepressants and helps restore emotional balance, improve mood, and reduce compulsive or anxious behaviours.

MECHANISM OF ACTION

Sertraline is a selective serotonin reuptake inhibitor (SSRI). It works by blocking the reabsorption of serotonin in the brain, increasing the availability of this neurotransmitter in the synaptic cleft. Higher serotonin levels improve communication between nerve cells and help regulate mood, anxiety, sleep, and emotional responses. Unlike older antidepressants, sertraline has minimal effect on other neurotransmitter systems, which gives it a more favorable side effect profile.

DOSAGE AND ADMINISTRATION

•      Adults (depression/OCD): 50mg once daily, may be increased up to 200mg/day

•      Panic disorder/PTSD/social anxiety: start at 25mg once daily, increase to 50mg after one week

•      PMDD: 50mg/day either continuously or only during the luteal phase

•      Tablets should be taken at the same time each day, with or without food

•      Full therapeutic effect may take 4 to 6 weeks

SIDE EFFECTS

Common:

•      Nausea, diarrhoea, dry mouth

•      Insomnia or increased sleepiness

•      Sweating, tremor

•      Sexual dysfunction (reduced libido, delayed ejaculation)

Less common:

•      Agitation, restlessness, or increased anxiety early in treatment

•      Weight changes

•      Hyponatraemia (low sodium), especially in elderly patients

•      Risk of serotonin syndrome when combined with other serotonergic drugs

NOT RECOMMENDED IN

•      Concurrent use with monoamine oxidase inhibitors (MAOIs) — allow 14 days gap

•      Known hypersensitivity to sertraline

•      Use with caution in patients with a history of mania or bipolar disorder

•      Not recommended in children under 6 years of age

DRUG INTERACTIONS

Sertraline should never be combined with MAOIs, as this can cause a potentially fatal serotonin syndrome characterised by high fever, agitation, muscle rigidity, and rapid heart rate. Concurrent use with other serotonergic agents such as tramadol, triptans, linezolid, or St John's Wort also raises this risk. Sertraline can increase plasma levels of tricyclic antidepressants and certain benzodiazepines metabolised by CYP2D6. Patients on warfarin should have their clotting time monitored more closely during sertraline therapy. NSAIDs, aspirin, and anticoagulants used alongside sertraline may increase the risk of gastrointestinal bleeding.
