INDICATION

Afdol 10mg Tab is prescribed for the treatment of moderate to severe dementia associated with Alzheimer's disease. It helps manage symptoms such as memory loss, confusion, and difficulty performing daily tasks. While it does not cure Alzheimer's disease, it can slow the progression of cognitive decline and improve a patient's ability to function in everyday life.

MECHANISM OF ACTION

Memantine works as a non-competitive N-methyl-D-aspartate (NMDA) receptor antagonist. In Alzheimer's disease, excess glutamate is released in the brain, causing prolonged activation of NMDA receptors and leading to neuronal damage through a process called excitotoxicity. Memantine blocks these overactive NMDA receptors, reducing the toxic effects of excessive glutamate while still allowing normal receptor function. This protective action helps preserve neuronal integrity and improve cognitive symptoms.

DOSAGE AND ADMINISTRATION

•      Starting dose: 5mg once daily in the first week

•      Week 2: increase to 5mg twice daily (10mg/day)

•      Week 3: increase to 5mg morning and 10mg evening (15mg/day)

•      Maintenance dose: 10mg twice daily (20mg/day) from week 4 onwards

•      Tablets may be taken with or without food

•      Dose adjustments required in patients with moderate to severe renal impairment

SIDE EFFECTS

Common:

•      Headache, dizziness

•      Constipation

•      Somnolence (drowsiness)

•      High blood pressure (hypertension)

Less common:

•      Confusion, hallucinations

•      Fatigue and general weakness

•      Vomiting, urinary tract infections

•      Abnormal gait or falls in elderly patients

NOT RECOMMENDED IN

•      Known hypersensitivity to memantine or any excipient in the formulation

•      Use with caution in patients with epilepsy or a history of seizures

•      Avoid in patients with severe renal impairment unless carefully dose-adjusted

•      Not recommended during pregnancy or breastfeeding without medical supervision

DRUG INTERACTIONS

Memantine can interact with other NMDA antagonists such as amantadine, ketamine, and dextromethorphan, potentially increasing the risk of adverse effects including psychosis. Drugs that alkalinise urine, such as carbonic anhydrase inhibitors and sodium bicarbonate, can reduce the renal clearance of memantine, raising its blood levels. Cimetidine, ranitidine, procainamide, quinine, and quinidine may share the same renal transport system and cause elevated plasma concentrations of memantine. Concurrent use with L-DOPA or dopaminergic and cholinergic agents should be monitored for additive or altered effects.
