INDICATION

Vilget-M 50mg+1000mg Tab is a dual oral antidiabetic combination indicated for the management of type 2 diabetes mellitus in adults who are inadequately controlled on metformin alone at the maximum tolerated dose, or in patients already stabilized on the individual components. It is used as an adjunct to diet and exercise to improve glycemic control and is suitable for patients requiring both incretin-based enhancement of insulin secretion and reduction of hepatic glucose output.

MECHANISM OF ACTION

Vilget-M 50mg+1000mg Tab combines vildagliptin and metformin for effective glycemic control. Vildagliptin, a DPP-4 inhibitor, prolongs incretin hormones (GLP-1 and GIP), enhancing glucose-dependent insulin release after meals and suppressing glucagon to reduce post-meal glucose spikes without causing hypoglycemia. Metformin lowers blood sugar by reducing hepatic glucose production and improving insulin sensitivity in muscle and fat. Together, they control both fasting and postprandial hyperglycemia while addressing insulin resistance.

DOSAGE AND ADMINISTRATION

•      Take one tablet twice daily, with morning and evening meals.

•      Taking with food reduces gastrointestinal side effects from metformin.

•      Swallow whole with a full glass of water; do not crush or split.

•      Maintain regular meal timing to optimize glycemic control.

•      Regular HbA1c and fasting glucose monitoring is advised throughout treatment.

SIDE EFFECTS

Vilget-M 50mg+1000mg Tab is contraindicated in type 1 diabetes mellitus and diabetic ketoacidosis. It must not be used in patients with renal impairment with eGFR below 30ml/min/1.73m², severe hepatic impairment, or conditions associated with tissue hypoxia and increased risk of lactic acidosis, including decompensated heart failure, respiratory failure, recent myocardial infarction, excessive alcohol intake, and dehydration. It is also contraindicated during pregnancy and breastfeeding.

DRUG INTERACTIONS

Coadministration with insulin or sulfonylureas increases hypoglycemia risk; lower doses of the secretagogue or insulin may be required. Glucocorticoids, thiazide diuretics, and sympathomimetics may worsen glycemic control and require dose adjustments. Iodinated contrast media can trigger acute kidney injury in patients on metformin; temporary discontinuation at least 48 hours before and after the procedure is recommended.
