INDICATION

Ternelin 2mg Tab is a centrally acting muscle relaxant prescribed for the management of spasticity and painful muscle spasms associated with neurological conditions. It is indicated in adults experiencing muscle hypertonicity related to multiple sclerosis, spinal cord injury, cerebral palsy, stroke, and other upper motor neuron disorders. MECHANISM OF ACTION

Tizanidine is an alpha-2 adrenergic receptor agonist that acts centrally in the spinal cord. Under normal conditions, motor neurons in the spinal cord receive signals from the brain that control muscle tone. In spastic conditions, these signals become overactive, causing muscles to remain in a constant state of contraction. Tizanidine works by stimulating alpha-2 receptors on the presynaptic nerve terminals in the spinal cord, reducing the release of excitatory amino acids such as glutamate and aspartate. This decreases the excessive nerve signals that drive abnormal muscle contractions, effectively reducing muscle tone and relieving spasm-related pain.

DOSAGE AND ADMINISTRATION

•      Starting dose: 2mg two to three times daily, taken orally with or without food.

•      Dose may be gradually increased by 2mg every three to four days based on clinical response.

•      Maximum recommended dose: 36mg per day, in divided doses.

•      Swallow tablets whole with water; take consistently at the same times each day.

•      Do not stop abruptly; taper the dose under physician supervision to avoid rebound spasticity.

SIDE EFFECTS

•      Common: Drowsiness, dizziness, dry mouth, fatigue, weakness, hypotension.

•      Less common: Elevated liver enzymes, hallucinations (rare), bradycardia, urinary frequency.

NOT RECOMMENDED IN

Ternelin 2mg Tab is not recommended in patients with known hypersensitivity to tizanidine. It must be used with extreme caution in patients with hepatic impairment, as the drug is extensively metabolized in the liver. Concurrent use with fluvoxamine or ciprofloxacin is contraindicated as these drugs dramatically increase tizanidine plasma levels. Tizanidine is not suitable for patients with severe renal impairment. Patients who operate vehicles or heavy machinery must exercise caution due to pronounced sedation.

DRUG INTERACTIONS

Fluvoxamine and ciprofloxacin are absolutely contraindicated with tizanidine, as they inhibit CYP1A2 and can increase tizanidine levels by up to 33-fold, causing potentially fatal hypotension and sedation. Antihypertensive medications have additive blood pressure-lowering effects with tizanidine; blood pressure should be closely monitored. Other CNS depressants, including benzodiazepines, opioids, and sedative antihistamines, worsen drowsiness. Oral contraceptives can reduce tizanidine clearance.
