INDICATION

Tacogen 1mg Capsules is a calcineurin inhibitor immunosuppressant indicated for the prophylaxis of organ rejection in kidney, liver, and heart transplant recipients. It is used as part of combination immunosuppressive regimens to prevent T-lymphocyte mediated immune rejection of the transplanted organ.

MECHANISM OF ACTION

Tacrolimus binds to the intracellular immunophilin FKBP-12, forming a complex that inhibits calcineurin, a phosphatase required for the activation of the transcription factor NFAT (nuclear factor of activated T cells). Blocking NFAT prevents the transcription of interleukin-2 and other cytokines essential for T-lymphocyte proliferation and activation, thereby suppressing the immune response against the transplanted organ. Tacrolimus is approximately 100 times more potent than cyclosporine on a weight basis in suppressing T-cell activation.

DOSAGE AND ADMINISTRATION

•      Dosage is highly individualized based on organ transplanted, body weight, and through blood levels

•      Typical adult kidney transplant: 0.2 to 0.3mg/kg/day in two divided doses

•      Therapeutic drug monitoring (trough blood levels) is mandatory throughout therapy

•      Always prescribed and adjusted by a transplant specialist; never self-adjust the dose

SIDE EFFECTS

Common:

•      Tremor

•      Headache and insomnia

•      Hyperglycemia and new-onset diabetes mellitus

•      Hypertension

Less common:

•      Nephrotoxicity (dose-dependent; monitor renal function regularly)

•      Neurotoxicity: seizures, encephalopathy, and leukoencephalopathy

•      QT prolongation

•      Increased susceptibility to infections and lymphomas with prolonged use

NOT RECOMMENDED IN

•      Concurrent use with cyclosporine (additive nephrotoxicity; allow 24-hour washout between agents)

•      Known hypersensitivity to tacrolimus or polyoxyl 60 hydrogenated castor oil (in intravenous formulations)

•      Pregnancy and breastfeeding unless absolutely necessary under specialist supervision

•      Grapefruit and grapefruit juice must be strictly avoided throughout therapy

DRUG INTERACTIONS

Strong CYP3A4 inhibitors including azole antifungals, macrolides, and protease inhibitors markedly increase tacrolimus blood levels, raising toxicity risk; frequent monitoring and dose reduction required. CYP3A4 inducers such as rifampicin, phenytoin, and St John's Wort dramatically reduce tacrolimus levels, risking acute rejection. NSAIDs combined with tacrolimus increase nephrotoxicity. Potassium-sparing diuretics and ACE inhibitors increase hyperkaliemia risk.
