INDICATION

Hilin 50mg Cap is a gabapentinoid anticonvulsant used in the management of neuropathic pain associated with diabetic peripheral neuropathy, postherpetic neuralgia, and spinal cord injury. It is also indicated as adjunctive therapy for partial-onset seizures in adults and in the treatment of generalized anxiety disorder and fibromyalgia.

MECHANISM OF ACTION

Pregabalin binds with high affinity to the alpha-2-delta subunit of voltage-gated calcium channels in the central nervous system. This binding reduces the influx of calcium into hyperexcited neurons, decreasing the release of excitatory neurotransmitters including glutamate, noradrenaline, and substance P. The result is reduced neuronal excitability, which underlies its analgesic, anticonvulsant, and anxiolytic effects.

DOSAGE AND ADMINISTRATION

•      Neuropathic pain: 50mg three times daily initially; may increase to 100mg three times daily within one week

•      Epilepsy (adjunctive): 50mg two to three times daily; titrate based on response up to 600mg/day

•      Generalized anxiety disorder: 50mg three times daily initially; adjust as directed

•      Always follow the titration schedule and dosage prescribed by your doctor; do not stop abruptly

SIDE EFFECTS

Common:

•      Dizziness and somnolence (most common; occur early in treatment)

•      Weight gain with prolonged use

•      Peripheral oedema (ankle and foot swelling)

•      Blurred vision and visual disturbances

Less common:

•      Euphoria and mood changes (abuse potential; monitor in susceptible patients)

•      Ataxia and incoordination

•      Dry mouth and constipation

•      Cognitive impairment and difficulty concentrating

NOT RECOMMENDED IN

•      Known hypersensitivity to pregabalin or any excipient

•      Severe renal impairment without dose adjustment (dose based on creatinine clearance)

•      Pregnancy and breastfeeding (insufficient safety data; discuss risk-benefit with doctor)

•      Do not stop abruptly; taper gradually to avoid withdrawal seizures and anxiety

DRUG INTERACTIONS

CNS depressants including opioids, benzodiazepines, alcohol, and sedating antihistamines have additive sedative and respiratory depressant effects with pregabalin; use with extreme caution. Oxycodone combined with pregabalin significantly increases the risk of respiratory depression and cognitive impairment. ACE inhibitors used concurrently may increase the risk of angioedema. Thiazolidinediones such as pioglitazone may worsen peripheral oedema and weight gain when combined with pregabalin.
